The actual chime mechanism, the guts of the chime so to speak, mounts directly to wall, and the cover fits over it.
The chime mechanism is a smaller, black box that contacts the doorbell strike plate, the plunger that strikes the plate, and were the wires are connected. The cover simply hangs over it.

The doorbell shipped right away, and hooked easily into our preexisting wiring! The white cover is a nice upgrade to the old bell we had hanging on the wall.
Added a Google doorbell to my house and wanted to retain the inside traditional chime, but mine was original to the house (mid century) and needed replaced.
